How to organise a Physiotherapy, Osteopathy or Chiropractic appointment funded by DVA
- Check the treatment is linked to your accepted condition
DVA funding generally applies where the treatment relates to an accepted DVA condition. White Card holders can access treatment for accepted conditions only. - Choose your provider
You can choose a registered:
• Physiotherapist
• Osteopath
• Chiropractor
They do not need to be specially “DVA approved”, but they must be appropriately registered and able to treat DVA clients.
Ask your GP or referring provider for a D0904 referral
The correct form is the DVA Referral Form — D0904. This form is used to refer directly to an allied health provider when prior approval is not required.
Make sure the referral includes the right details
The D0904 should include:
• your DVA file number
• card type
• accepted disabilities
• condition being treated
• clinical details
• provider you are being referred to
• referring provider details and signature
Book your appointment
Contact the physiotherapist, osteopath or chiropractor directly and let them know you are a DVA client.
Provide the D0904 to the provider
Give the completed referral form to the clinic before or at your first appointment.
Start your treatment cycle
Under DVA’s treatment cycle arrangements, an allied health provider may treat a client for up to 12 sessions or one year — whichever ends first.
Review if more treatment is needed
At the end of the treatment cycle, the provider reports back to your usual GP. If further treatment is clinically needed, your GP can provide another referral.
Quick takeaway:
Choose a registered physio, osteo or chiro → get a D0904 referral → give it to the provider → begin treatment under DVA’s treatment cycle.
